The initial assessment

The first home visit involves a comprehensive assessment by a trained nurse using the InterRAI-HC instrument, recognized by the Swiss Home Care Association. This assessment considers not only clinical aspects but also the family context, habits, and available resources.

Based on the initial assessment, we develop a treatment plan that includes:

The REFERENCE nurse, under the supervision of the Medical Director, develops a comprehensive care plan based on the initial assessment.

Quality and constant adaptation

We carry out comprehensive reassessments every six months through the InterRAI-HC tool, or more frequently in the event of significant changes. This allows us to adapt the treatment plan to the user’s needs while maintaining the expected quality standards.
