The initial assessment
The first home visit involves a comprehensive assessment by a trained nurse using the InterRAI-HC instrument, recognized by the Swiss Home Care Association. This assessment considers not only clinical aspects but also the family context, habits, and available resources.
Complete needs assessment
Based on the initial assessment, we develop a treatment plan that includes:
- Needs assessment through Cantarelli model
- Nursing diagnoses according to NANDA Taxonomy
- Documentation collection and authorization
- Preliminary definition of the care plan
Personalized Care Plan
Definition of objectives and interventions
The REFERENCE nurse, under the supervision of the Medical Director, develops a comprehensive care plan based on the initial assessment.

We carry out comprehensive reassessments every six months through the InterRAI-HC tool, or more frequently in the event of significant changes. This allows us to adapt the treatment plan to the user’s needs while maintaining the expected quality standards.
